FROM HOSPITAL OR REHAB TO HOME

Planning home health after discharge

Returning home can bring questions about nursing, wound care, medications, and therapy. Contact Epic while discharge is being planned so intake can review the requested services and the documents needed for a coordinated handoff.

Our Reston-based intake team reviews requests in Fairfax, Reston, Alexandria, Arlington, Woodbridge, and Loudoun County. Availability depends on the home address, requested service, payer requirements, and staffing. Check our service areas or call 571-249-5819.

For patients and family caregivers

Have the expected discharge date, home ZIP code, insurance plan name, and ordering provider’s contact information available when you call. Explain whether nursing, therapy, personal care, or more than one service has been requested.

For discharge planners and referring offices

Call 571-249-5819 and fax referral records to 571-260-0165. Include provider orders, relevant clinical notes, demographics, insurance information, and the requested service and start date. Our team will review missing information and confirm whether the referral can be accepted.

Confirm care before making arrangements

Sending records does not confirm acceptance or a visit time. Intake reviews clinical needs, provider orders, coverage, authorization requirements, location, and staffing. We confirm the next steps after those reviews.

Questions about home health after rehab

Is a hospital stay required for Medicare home health?

A hospital stay is not required just to qualify for the Medicare home health benefit. Coverage has other requirements, including an eligible need for intermittent skilled services, homebound status, and provider assessment and orders. Your care team and intake review the individual circumstances.

Can nursing and therapy be part of the same plan?

They may be, when ordered and clinically appropriate. Intake reviews the requested disciplines, coverage, and staffing. Therapy availability varies by location; Purcellville is currently nursing only.

Does Medicare pay for ongoing personal care alone?

Medicare’s home health benefit does not cover personal care when it is the only care needed. Personal care and Virginia Medicaid LTSS follow different eligibility and authorization processes. Ask intake which service pathway fits your request.

How soon can the first visit happen?

Tell intake your anticipated discharge and requested start date. Timing depends on completed records and orders, coverage or authorization needs, clinical review, and available staff. A start date is confirmed individually.
